**An Original Song -- Created by YOU!**

 

Applicants much be 25 years of age or younger

*City where you're from*

*Phone number for the main contact person*

*Band Name & Genre of Music*

*Cover Song you will be performing*

*YouTube link to video of your band performing*

 

THE WINNER WILL RECEIVE A CASH PRIZE OF $500!!

PLUS an extended First Night performance after the contest!

Each band member will receive free entry to First Night Youngstown.

In the event that band members are under the age of 13, one parent per child will receive free entry.
